Requirements for New Students
Health Services seeks to ensure a safe, healthy environment for the entire campus community. In order to do so, there are forms that are required of every undergrad and seminaian which you will find for downloading below. Please read everything carefully and submit the information by your Student Orientation date.
Meningococcal disease is a potentially life-threatening bacterial infection. The disease is most commonly expressed as meningococcal meningitis, an inflammation of the membranes surrounding the brain and spinal cord, or meningotoccemia, the presence of bacteria in the blood. Recent research has shown that students living in residence halls, particularly first-year students, are at a higher risk than university students overall.
Maryland law requires that an individual enrolled in an institution of higher education in Maryland and residing in university housing must be vaccinated against meningococcal disease. The student (or guardian if under the age of 18) may choose to waive the vaccine if he or she has reviewed information regarding the risks of meningococcal disease and all students are to sign the meningitis waiver form (Page 4) that he/she has chosen to waive the vaccination.
