Residence Life Downloadable Forms 1. Party Reservation Form (PDF) Before hosting a party, students must fill out this form and have it approved by the Office of Residence Life. This form reviews the definition of a party and party guidelines. Parties may be reserved for Fridays and Saturdays. Reservations may be made beginning on Tuesday at 1 p.m. and must be made by Friday at 12 noon of the respective weekend. 2. Special Needs Housing Requests (PDF) If you have special housing needs, you must fill out all three of the three Special Needs Forms. Requests for special housing are reviewed on a case by case basis. Submitting a request does not guarantee that your application will be approved. Students with the most severe needs will be given first priority in securing special housing if their need/disability significantly impacts an essential life function. Special Needs Form 1: Student Request for Special Housing Accommodation: This form must be completed by the student. This form must be submitted each academic year that the student plans to live on-campus. Special Needs Form 2: Documentation of a Medical Disability: This form must be completed by a Doctor/Health Professional. This provides detailed information so that the Special Housing Review Group can make an appropriate decision. Special Needs Form 3: Student Request for Air Conditioning: This form must be completed by a Doctor/Health Professional. This form is for students who need air-conditioned housing because of a medical reason.
3. Roommate Agreement (PDF) This form is designed to assist students in the formation of positive roommate experiences. University students are treated as adults and are expected to put forth the necessary effort to ensure a comfortable living environment for themselves and their roommate(s). 4. Housing Contract 2009-2010 (PDF) This is the housing contract for the 2009-2010 academic year. 5. Request for Release from Housing Contract (PDF) This form must be completed and turned into the Residence Life office if you break your housing contract. This form allows you to be considered for approval to be released from the fine. It does not guarantee that you will not have to pay the fine for breach of contract stated in the Housing Contract (see link above). 6. Residence Life Incident Form Any member of the Mount community may refer allegations of student misconduct that may be in violation of the Code of Student Conduct or Community Standards. The allegations should be written on this form. Persons making such allegations are required to provide information pertinent to the case and may be expected to appear during judicial conference as the person making the charge. This is not confidential. Confidential reports can be made through Public Safety's Silent Witness Statement. 7. RA Program Peer Evaluation This form allows the Residence Life staff to evaluate our progress as it relates to RA programming. Your feedback will help us to provide entertaining, worth while, educational programs for you and your residents. All responses are kept confidential and will only be used to help improve programs for the future 8. Interim Housing Request Form This form outlines your responsibilities during a break period which you agree to abide by during the days you are approved to stay on campus. |
