Residence Life Forms
1. Party Reservation Form (PDF)
Before hosting a party, students must fill out this form and have it approved by the Office of Residence Life. This form reviews the definition of a party and party guidelines. Parties may be reserved for Fridays and Saturdays. Reservations may be made beginning on Tuesday at 1 p.m. and must be made by Friday at 12 noon of the respective weekend. Please note that change to 2011-2012 is that all residents of a room must be 21 years of age to submit a part reservation request.
2. Special Needs Housing Requests (PDF)
If you have special housing needs, you must fill out all three of the three Special Needs Forms. Requests for special housing are reviewed on a case by case basis. Submitting a request does not guarantee that your application will be approved. Students with the most severe needs will be given first priority in securing special housing if their need/disability significantly impacts an essential life function.
