New Student Orientation Forms All Students need to fill out the following forms. Registration for Orientation is closed. If you have any questions concerning registration please email orientation at orientation@msmary.edu . Please be sure to fill out ALL of the forms below, every incoming student must fill these out. Campus Dining Form All students living on campus are required to have a meal plan, the Campus Dining Form must be filled out to get that meal plan. Visual and Performing Arts Survey We offer a wide variety of Visual and Performing Arts opportunities, all students should fill out this form to learn about all that is offered and let us know your areas of interest. Since all medical forms require signatures these can be downloaded as PDF's to be printed and brought to the June Orientation.
