Requirements for New Students

Health Services seeks to ensure a safe, healthy environment for the entire campus community. In order to do so, there are several requirements that every undergraduate student and seminarian must meet in order to attend the Mount. Please read everything carefully and submit the information by your Student Orientation date. These forms are available at the Health Services or can be downloaded at the bottom of this page.

• Health history (must be completed on the Mount form)

• Physical and Immunizations (to have been done within one year prior to attending the Mount)

• Meningococcal Vaccine Form (required by Maryland State law for every resident student)

Meningococcal disease is a potentially life-threatening bacterial infection. The disease is most commonly expressed as meningococcal meningitis, an inflammation of the membranes surrounding the brain and spinal cord, or meningotoccemia, the presence of bacteria in the blood. Recent research has shown that students living in residence halls, particularly first-year students, are at a higher risk than university students overall.

Maryland law requires that an individual enrolled in an institution of higher education in Maryland and residing in university housing must be vaccinated against meningococcal disease. The student (or guardian if under the age of 18) may choose to waive the vaccine if he or she has reviewed information regarding the risks of meningococcal disease and signed a written waiver that he/she has chosen to waive vaccination.

For undergraduate students and seminarians entering the Mount in the fall, the following forms are due at Student Orientation in June. If you will not be attending Orientation, please mail your forms to:
