This form must be completed and submitted within the first three weeks of each semester for consideration of funding through SGA. Please note that allocation of funds can be tied to fulfillment of the service requirement, submission of semester reports, scope of the club’s impact for the Mount community, and adherence to all SGA and Campus Activities policies and procedures.
