Admissions to the Seminary

General Information
Mount St. Mary's Seminary requires that all candidates for admission complete our official application form (even though priesthood candidates may have to complete a similar form for their sponsoring diocese) and submit all of the documentation listed in the application packet. 

For your convenience, the application materials are available in a fillable pdf format by using the links at the left.  Please note that Lay Students (non-seminarians) should use the short version.

Follow all directions carefully, and review the documents before printing all required pages - you will not be able to electronically save your responses on the pdf form.

All degree candidates must submit official transcripts from all colleges, universities, or seminaries previously attended. Each institution must be contacted in writing by you, the student, to request a transcript copy. For your convenience, Click Here to down-load a sample "Transcript Request" form letter.
