General Information
An official application form may be obtained directly from the seminary Admissions Office or from the vocation director’s office of the sponsoring diocese.  For your convenience, pdf copies of the application materials are available by using the links at the left.  Please note that Lay Students (non-seminarians) use a short version.

All degree candidates must submit official transcripts from all colleges, universities, or seminaries previously attended. Each institution must be contacted in writing by you, the student, to request a transcript copy. For your convenience, Click here to down-load a "Transcript Request" form letter.
