| Requirements for New Students Health Services seeks to ensure a safe, healthy environment for the entire campus community. In order to do so, there are several requirements that ever student must meet in order to attend the Mount. Please ready everything carefully and submit the information by the due date(s) listed. These forms are available at the Wellness Center or can be downloaded at the bottom of this page. - Health history (must be completed on MSM form)
- Physical and Immunizations (done within one year prior to attending the Mount)
- Meningococcal Vaccine Form (required by Maryland State law for every resident student)

Meningococcal disease is a potentially life-threatening bacterial infection. The disease is most commonly expressed as meningococcal meningitis, an inflammation of the membranes surrounding the brain and spinal cord, or meningotoccemia, the presence of bacteria in the blood. Recent research has shown that students living in residence halls, particularly first-year students, are at a higher risk than university students overall. Effective June 1, 2000, Maryland law requires that an individual enrolled in an institution of higher education in Maryland and residing in university student housing must be vaccinated against meningococcal disease. The student (or guardian if under the age of 18) may choose to waive the vaccine if he or she has reviewed information regarding the risks of meningococcal disease and signed a written waiver that he/she has chosen to waive vaccination. For students entering the Mount in the fall of 2008, the following forms are due no later than June 23, 2008.
