Students: Application for College Major: This form is to be used to declare your major by end of sophomore year. Download Application for Minor: If you want a minor to appear on your transcript, complete this form by senior year. Download Application for Area of Concentration: (Bus/Acctg/Econ) Business, Accounting & Economics Department majors: if you want a concentration to appear on your transcript, complete this form by senior year. Download Application for Transfer of Credits: Complete and submit this form to make sure that courses you take at another institution are the ones you need and that they will transfer for credit to the Mount. Download Foreign Study: Complete and submit this form to make sure that courses you take while studying abroad at another institution are the ones you need and that they will transfer for grade and credit to the Mount. Download Application for Academic Waiver: Students seeking waiver of a requirement in the core curriculum or major should complete and submit this form. Download Pass/Fail Authorization: Students seeking to take a course on a pass/fail basis instead of with a regular letter grade should complete and submit this form. Download
Faculty: Grade Change Form: Form to be used by faculty to submit grade changes to the registrar’s office. Download Incomplete Form: Form to be used by faculty to submit a grade of “I” to the registrar’s office for students who need an incomplete. Download Permission to Release Educational Records: Form to be used by faculty to obtain persmission from students to release specific educational record information for the purposes of recommendation letters, conversations with parents, etc. Download
Students and Faculty: Application for Independent Study/Tutorial: A two-page learning agreement form to be completed by student and faculty mentor for an independent study or tutorial. Download
